The Country Dental plan provides x-rays exams, office visits, cleaning and other preventive dental services for the entire family.

All members will enjoy a 25% discount on all dental and cosmetic procedures preformed in our office and a 10 % discount of orthodontic.

The most important component in dental care without exception is prevention and this is the most valuable service we can offer you. Through our prevention control program we successfully recall patient every 6 months for check-ups, allowing us to easily address any potential problem in order to avoid restorative treatment.

Exclusions & Limitations:

• Discounts are only available for services performed at our facility; referrals to specialists are not a covered benefit. (On average, less than 1% of all our patients will need to be referred out).
• Prescription medications are not covered.
• There will be a $75 charged for missed appointments without 24 hours notice.
• This plan is not to be in connection with any Workers Compensations Plan or State Aid.
